New to the practice

We invite  invite you to sign up for your personal online patient account by visiting our patient portal and requesting an activation code. The code will be emailed to you via our secure system. This code allows you to create your new online patient account, user ID and password. Within this account you can fill out all of your new patient forms so you will be ready for your first appointment.

 On Appointment Day

To help us expedite your office visit, please arrive 15 minutes prior to your scheduled appointment.  This allows our staff time to make any updates to your medical history or insurance information.

Please bring your insurance card, list of current medication and any co-pay you may have to your appointment. This helps us process your insurance more quickly and provide you better service.


We apologize when we fall behind but emergencies do occur and we do our best to accommodate all of our patients. You are welcome to call ahead to see if we are running on time.

Cancellations

If you are unable to keep your appointment please notify us 24 hours before your scheduled time. This allows us time to accommodate another patient
